Do I need a referral from my primary care physician?
Generally, you will not need a referral from your primary care physician (PCP) to see us. However, in rare cases, HMO-type insurance plans will require you be referred to us by your PCP before they will cover your treatment. If you have insurance, it is always wise to check with them prior to your treatment at our office.
What type of insurance do you accept?
We accept Blue Cross Blue Shield, Aetna, Humana and some PHCS plans. If we are not in-network with your insurance company, please contact our office as discounts are available to self-pay patients who pay at the time of service.
What do I need to bring to an initial exam?
1. X-rays, MRI reports, consultation notes from other physicians or any other relevant information that you have already obtained as it relates to your current condition…these are helpful but not absolutely necessary.
2. Contact information for your regular doctor if you would like us to coordinate care with them.
3. Current ID and any applicable insurance cards.
3. Payment method (payment is expected upon completion of the exam).
How often will I need to come in to see the doctor?
Your treatment plan/number of visits varies based on your condition. When you come in to see Dr. Olsen, he will set up a treatment plan that will achieve the best results for you, based on his findings. No two patients are exactly alike. Likewise, treatment plans vary greatly from patient to patient. A treatment plan will be discussed with the patient before treatment begins. This treatment plan is based on what the doctor thinks it will take to get you in good health. The treatment plan is not “set in stone”, but serves as a good guideline.
How much does an exam cost?
This varies based on your condition. We can provide you with an estimate for any procedure done outside of a standard examination before any work is performed. Please feel free to contact our office with specific questions.
What forms of payment do you accept?
We accept Visa, MasterCard, Discover, American Express, CareCredit, personal checks, and cash. Payment is due in full at every visit.
Does the doctor make house calls?
At this time, the doctor does not make house calls.
